Patient Values

Creating Patient Values for CKHA:

We would like to hear from you. You are invited to read a draft Patient Declaration of Values document, created by the Patient and Family Centred Care Steering Committee, with patient and family advisors.

After reading the document, we invite you to complete the following 5 question survey. Your comments will be considered in the creation of a final Patient Declaration of Values document for CKHA.  Thank you for taking the time to help us in the creation of a patient values document that reflects what is most meaningful to our patients and families.

In addition to the survey, you are invited to visit a kiosk on the following dates and locations:

February 8th - 1100-1600 hours - Sobeys Wallaceburg          

February 10th - 1000-1600 hours - Downtown Chatham Centre

February 14th - 0900-1100 hours - Streetscape  at CKHA - Chatham

February 24th - 0900-1100 hours - CKHA front lobby - Wallaceburg

The Excellent Care for All Act, legislated by Ontario's Ministry of Health and Long-term Care, puts patients first. By June 2011 all hospitals will have a Patient Declaration of Values developed with input from the public.
