Indigenous Health
The Indigenous Transition Navigator (ITN) is a position that works at CKHA’s Chatham and Wallaceburg Sites. The ITN is available to support Indigenous patients and their families during their stay at CKHA, with the goal of helping to make hospital care safe, comfortable, and less confusing.
The goal of the ITN is to help make your hospital care and journey to wellness safe, comfortable, and less confusing.
Anyone who self-identifies as Indigenous and is interested in receiving these services can speak to their health care team for a referral. Click here to learn more.
