CKHA Quality Balance Scorecard
Quality Improvement Plan
At Chatham-Kent Health Alliance (CKHA), our community is at the heart of everything we do. Under our guiding philosophy, “Our Focus is You,” we are dedicated to providing easy-to-access healthcare rooted in equity, kindness, and safety.
Quality is a shared responsibility among our staff, physicians, and volunteers. Our Quality Improvement Plan (QIP) reflects our commitment to maintaining the highest standards of patient safety and service excellence. The QIP is an annual, publicly available document mandated for healthcare organizations under the Excellent Care for All Act (2020). The expectation is for health care organizations to have their QIPs in place, publicly posted, and submitted to Ontario Health by April 1 every year. Planning for the annual QIP takes place in the fall and is a collaborative effort. Patient and Family Advisors, frontline staff, and physicians are engaged to ensure the goals we set reflect the real-world experiences of those giving and receiving care. These living documents have multiple components and their purpose is to outline specific quality-based goals, targets, and actionable strategies to continuously improve quality care; with a focus on access, safety, equity, and experience.
