CKHA @ Home is an intensive, hospital-operated homecare service designed to bridge the gap between acute care and a patient’s home. By providing a seamless transition, we aim to ensure patients continue to progress their functional independence upon discharge, thereby reducing the risk of future readmissions.
There is an 8-week and a 16-week bundle available through the Hospital 2 Home (H2H) Program. We are proud to partner with ParaMed Home Health Care, our primary service provider in the community, to deliver this high-level, integrated care — bringing hospital-quality support directly into patients’ homes.
