Diabetes Education Centre

The Diabetes Education Centre provides care to patients with scheduled appointments out of several outreach sites throughout Chatham-Kent. The Diabetes Educators provide support to adults and children who have been diagnosed with diabetes, and to individuals and populations who are at higher risk of developing diabetes.

The Diabetes Education Centre interdisciplinary team consists of Certified Diabetes Educators (Registered Nurse Educators and Registered Dietitians specifically trained to manage all diabetes types), Primary Care Practitioners and Administrative Personnel.
