Rehabilitation & Integrated Stroke Unit

Chatham-Kent Health Alliance’s (CKHA) comprehensive Rehabilitation and Integrated Stroke Unit provides a continuum of care, incorporating both high and low intensity rehabilitation models, to maximize functional independence and improve quality of life for patients recovering from stroke, orthopedic conditions, and general debility. This unit caters to stroke patients and post-acute patients requiring goal-oriented therapy, with a mix of high and low intensity rehabilitation dependent on individual patient needs and tolerance.

An interdisciplinary team provides individualized treatment plans, patient and family education, and collaborative discharge planning, focusing on achieving optimal patient outcomes and a safe return to the community. All patients are assigned a team contact person (TCP) during their stay to ensure frequent and timely communication between the care team and patient & families.

The following healthcare professionals are a part of the Rehabilitation Team: Clinical Manager, Physician, Nurse, Nurse Practitioner, Physiotherapists (PT), Occupational Therapists (OT), Speech and Language Pathologist (SLP), Therapeutic Recreation, Social Work, Dietitian, Ontario Health at Home Care Coordinator, Volunteers and patients.
