Rehabilitation & Integrated Stroke Unit

The Chatham-Kent Health Alliance – Chatham Site, is one of five designated District Stroke Centres in Southwestern Ontario. As the designated District Stroke Centre for Chatham-Kent, CKHA’s Stroke team works collaboratively with many local organizations and disciplines to plan and implement strategies for improving stroke care for residents of Chatham-Kent. 

CKHA has been Accredited with Stroke Distinction through Accreditation Canada since 2022 reflecting the ongoing commitment to provide best practice stroke care.  

Services offered at the Chatham Site include: 

• Hyper Acute Stroke Services: The Chatham Site delivers clot busting medication to eligible patients which is time-sensitive and can significantly reduce damage cause by an ischemic stroke and improve patient outcomes. CKHA collaborates with partners including EMS and Telestroke to ensure that patients arrive and are assessed in a timely manner. Patients that may be eligible for mechanical clot removal (EVT) will be transferred to our Regional Stroke Centre – London Health Sciences Centre. 

• Urgent TIA Clinic/Secondary Stroke Prevention Clinic (SSPC): This clinic provides care to patients who are at risk of a stroke or have experienced a Transient Ischemic Attack (TIA) and are referred by their physicians. The clinic also provides care to patients who have experienced a stroke and need additional education and support to prevent another stroke in the future. The goals of the clinic include stroke prevention and risk management. The clinic is located in the Diagnostic Imaging area of the Chatham Site. 

• Integrated Stroke Unit: A dedicated, multidisciplinary team provides evidence-based care to stroke patients for both Acute and Inpatient Rehabilitation needs. Being an inpatient is just the beginning of a rehabilitation journey post-stroke that continues even after discharge. This Unit is located on the 3rd floor in Zone D. 

• Transitional Stroke Program: An outpatient rehabilitation program that delivers individual and/or group therapy for those recovering from stroke or other neurological conditions. Disciplines include: occupational therapy, physiotherapy, and speech language pathology.
