Core Programs

We provide a comprehensive program for women and children that includes Obstetrical triage-intake and registration, labour and delivery, nursery and pediatric services, including surgical day care.

In order that we provide you and your baby with the best care possible, we ask that you complete a pre-registration questionnaire. This questionnaire can be found HERE. If you prefer, you can request a copy from your delivery care provider or contact us to request a copy to be mailed to you (by phone at 519.352.6400 x 6707). We ask that you bring the completed questionnaire with you when you come for an appointment at the hospital or when you arrive for the birth of your baby. We will accept your completed form at the Women & Children’s main desk and your completed questionnaire will become part of your health record. Protecting your privacy is important to us and our privacy policy outlines our rules to ensure the information you provided us stays private.
