Discharge Planning

Discharge is a planned event that involves the patient and the family. Your doctors, nurses and other healthcare professionals help you co-ordinate your care so that you are well prepared in advance to return home as soon as possible. You are encouraged to ask questions and seek clarification if you do not understand something. Chatham-Kent Health Alliance works with many community resources to provide follow up during your recovery when you leave the hospital. Please ask for a copy of any discharge instructions that are important to you or any follow up appointments that you will need help remembering.
